Job Description Applications are invited for the position of full time Social Worker to work at Extendicare Cedars Villa our 243 bed Long-Term Care Home located in SW Calgary, Alberta. Reporting to the Executive Director, the Social Worker provides social work services for residents and families within legislative and budgetary parameters. RESPONSIBILITIES Participates in resident admission process including tours, admission, transfers, discharge, and occupancy tracking. Acts as liaison with families and advocate for residents to ensure they are allowed and encouraged to exercise their rights Participates in the interdisciplinary team process of resident assessment, care planning and evaluation to ensure services are planned according to individual resident and family needs.…
