Department: 39122 South Metro A - Home Care Physical Therapy Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: Full Time Home Health Physical Therapist Case Manager for South East Chicago. Normal Office Hours Monday Thru Friday 8.00 AM to 5.00 PM. Position includes weekend coverage rotation every 4th to 5th weekend and 2 Advocate holidays per year based on Departmental Needs. Pay Range: $44.15 - $66.25 PHYSICAL THERAPIST (PT) - HOME HEALTH CASE MANAGER LOCATION: CHICAGO SOUTHEAST SIDE HOURS: DAYS WITH ROTATION TO WKDS AND HOLIDAYS This position is eligible for up to a $10,000.00 sign on bonus.
