About this role
Working in the Office of the CEO and reporting to the VP of Point of Care & Risk Stratification, the Transformation Manager will support high-priority transformation initiatives, to aid in Aledade’s performance and growth. This role will initially focus on scaling the delivery and increasing the impact of value-based care insights into partner practice EHR workflows, which is Aledade’s priority transformation initiative in 2026. The team member will support projects spanning analytics, tool deployment, practice adoption, impact evaluation, and operations.
