Baystate Health, is looking for experienced Primary Care Nurse to join Northampton Family Medicine Full Time -40 hours Sign On Bonus eligible Responsibilities Provides independent quality triage and patient care utilizing strong assessment skills, point of care testing and quality assurance checks Provides education, ongoing assessment and support to patients with chronic substance use health conditions In partnership with patient and medical staff, develop and implement plan of care for complex patients including medical and/or psychosocial disorders.
