POSITION SUMMARY: Works with individuals served by BMC’s Community Behavioral Health Center (CBHC) to provide case management and health system navigation to improve coordination and outcomes. This role functions as part of a multidisciplinary team and provides direct communication to individuals receiving services to collaborate on treatment goals and provide updates on progress toward meeting these goals.
