Position Overview The Case Manager plays a vital role in supporting clients throughout their treatment journey by providing comprehensive case management services. This includes assisting with discharge planning, resource linkage, documentation, and advocacy. The ideal candidate will be organized, empathetic, and equipped with the knowledge to support clients with a variety of social, financial, and behavioral needs.
