Adult Partial Care Case Manager

Shore Haven provides treatment and support for adults who live with a severe and persistent mental illness as their primary diagnosis. Often times participants entering the program struggle with other co-occurring diagnoses, such as a particular substance use disorder. The Shore Haven program provides treatment six (6) hours each weekday, from 9am-3pm. Transportation arrangements can be made to accommodate program attendance. Partial Care day treatment services exist for those people who find it difficult to maintain their lives in the community because of a behavioral health illness.
