SUMMARY: The Clinical Care Liaison, under the supervision of the Transfer and Access and LifePACT Clinical Manager (or designee), collaborates with the EpiCenter team to enhance care accessibility within the Lifespan system. This role entails the facilitation, and when appropriate, the initiation of communication between practitioners within any of the institutions within the Brown University Health system to coordinate care and service, including but not limited to direct admissions, management of inpatient to outpatient clinic appointments, community paramedicine visits, and ambulance transport.
