Dyer Health Center South 2150 Gettler St Dyer, Indiana 46311 The PACE Driver is responsible for providing safe transportation to participants in a timely, competent manner and communicating information, relevant to the plan of care, with the interdisciplinary team (IDT). This position reports directly to the PACE Transport Supervisor. PACE drivers are required to demonstrate the knowledge and skills necessary, to provide service to frail elderly participants according to assigned responsibilities and (PACE Program) standards. The driver has direct interaction with caregivers and contracted partners, while conducting business on behalf of the Franciscan Senior Health and Wellness (FSHW) Program of All-Inclusive Care for the Elderly (PACE) program.…
