Transplant Coordinator III (NP) - Pulmonary Hypertension Cl - Full Time 8 Hour Days (Exempt) (Union)

The Transplant Coordinator III-NP, under the direction of a collaborating physician, assists in the care of patients with a variety of end stage organ diseases. Coordinates and participates in the medical/surgical management in either inpatient and/or outpatient setting of transplant and/or end stage organ diseases. Performs comprehensive history and physical exams, develop differential diagnoses, implement plan of care and document patient's responses. Order medications, diagnostic studies, and therapeutic procedures related to their illnesses and track results timely as outlined. The Transplant Coordinator III-NP also assists other personnel to ensure high quality delivery of patient care and services to a transplant/end stage organ disease patient population.…
