Physician - Primary Care --------------------------------------------------------------- This position provides medical care and treatment services with emphasis on chronic disease management and promotion of health and palliative care for patients not best served in a clinic setting. Additionally, this position provides active, consultative, and supportive assistance to non-physician providers as needed and/or as directed by the Medical Director. Care is provided in patient residencies, as well through audio and video sessions.
