Social Worker - Hospice - PRN --------------------------------------------------------------- The Hospice Social Worker is responsible for delivery of social work services to hospice patients, including psychosocial evaluations, ongoing counseling, casework services, coordination of referrals and bereavement services. Job Description Summary NOTE: This is a PRN role that will work 100% in the community and support the Portland Metro area. Candidates will need to have 5 weekdays of availability per month.
