Care Manager (RN)

Position Purpose: Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families. Key Details: This role is responsible for member outreach, case management enrollment, ongoing member management, and participation in clinical rounds. The ideal candidate will have disease management experience, strong clinical judgment, Microsoft proficiency, and a background in physical health case management.
