Job Description: This is a clinical role. The Community Care RN is responsible for delivering medical care in the communities where Cityblock members live. In homes, shelters, offices, public places or in the street. The Community Care RN is primarily responsible for facilitating virtual provider visits with Cityblock members and following medical orders, performing clinical skills and tasks within their scope, training, and policy.
