Who We Are Concert Health is a mission-driven behavioral health medical group delivering evidence-based Collaborative Care Model (CoCM) services in primary care. Our Concert Care Psychiatry program provides essential support for patients with complex adult ADHD diagnostic and short-term treatment needs, enabling primary care providers (PCPs) to confidently manage adult ADHD within the integrated care framework.
