DSD Services teammates will have access to all the capabilities and resources DaVita has to offer. The schedule is likely to be 3 days per week with NO on-call. The primary location is at DaVita SNF Dialysis at Haven Sky Harbor at 1880 E Van Buren St Phoenix, Az 85006. The CCHT must be willing to support other DaVita SNF Dialysis dens if needed.…
