Job Description A bit about the role... Devoted Medical was founded on the belief that if we treat each patient like we would our loved ones, we can meaningfully improve healthcare experiences and outcomes for some of America’s most vulnerable patient populations. This specific role provides social work case management support to our Specialty Care Programs: Diabetes, Hypertension, Congestive Heart Failure, and Chronic Obstructive Pulmonary Disease Management. This role will complement our interdisciplinary teams by addressing the psychosocial needs of our patients.
