Job Description A bit about this role: We want to help members navigate the healthcare system in a better and safer way. The Transitions of Care (TOC) Team supports members who are newly discharged from the hospital (and in some cases prior to discharge). You’ll serve as an advocate for these members, coordinating care and ensuring they have the necessary resources and support to achieve better health outcomes. As a TOC Associate your goal will be to support members in safely recovering at home and to prevent hospital readmissions.
