Job Description A bit more about this role: This is an exciting opportunity for a compassionate and skilled Advanced Practice Provider (APRN or PA) to provide virtual primary care to a panel of patients through our innovative telehealth platform. As a Primary Care Advanced Practice Provider (Longitudinal Chronic Disease Management), you’ll manage that panel of patients on their primary care journey with Devoted Medical. You’ll conduct comprehensive clinical assessments, develop plans for chronic condition management, and work on preventative and proactive care. You’ll collaborate closely with specialists, pharmacists, nurses, care coordinators, caregivers, and families to deliver coordinated, member-centered care.…
