Job Description A bit about this role Transitions of Care (TOC) is one of Devoted Medical's largest clinical programs, responsible for making sure thousands of Devoted members get home safely after a hospitalization. From admission through 30 days post-discharge, our virtual interdisciplinary teams deliver personalized, comprehensive wrap-around care that breaks down the clinical, social, and logistical barriers to post-discharge stability.
