General Purpose of Job: Provided by a trained and licensed professional, the Licensed Clinical Social Worker reports to the Director of Clinical Practice and Therapeutic Arts or the Care Coordination Manager. As a direct care provider, will provide a wide range of services to include; emotional support, supportive, goal directed, and therapeutic counseling, education, screening, assessment of patient and family needs, treatment planning, safety planning, and referral to community resources as they address the social and emotional aspects of a patient’s illness and the impact of illness on the patient and family.…
