This position is for our Extendicare Athabasca Home, located in Athabasca, Alberta Available opportunities include: 1 Full-Time Days and Nights rotation What you’ll be doing Liaise with interdisciplinary team and care providers to develop, implement, review and evaluate resident complex care plans including education and support.
