What You’ll Do Assist Population Healthcare Coordinators and leadership in implementation of Population Healthcare Programs (i.e. Anthem, Humana, UHC, Aetna, Optum) with focus on direct patient engagement, chart review and data submission, communication with physician office staff in coordinating care management. Responsible for assisting in implementing selected programs and providing continued feedback of programs to Population Healthcare and Business Development leadership.
