About this role
Wat ga je doen? Je zorgt als (klinisch) casemanager voor afstemming, coördinatie en uitvoering van zorg rondom de cliënt, zowel bij geplande zorg als in acute crisissituaties. Je werkt op de locatie in Dongen, een klinische ART afdeling. Je zorgt samen met het team voor de implementatie van het ART-model. Je coacht teams en individuele medewerkers bij hun werkzaamheden in het primair proces, op teamontwikkeling, ART methodiek en in hun professionele ontwikkeling. Je bent onderdeel van het behandelteam en werkt nauw samen met o.a. de teamleider en senior verpleegkundigen. Je levert vanuit een eigen professionele houding een bijdrage aan team overstijgende ontwikkelingen en projecten.…
