About Healthcare Talent is assisting our client in their search for a Medical Case Manager in the Covina, San Bernardino, Orange and Northridge areas. This is a Care Management position in an advanced specialty collaborative practice, responsible for providing ongoing case management services for members. The Case Manager provides intensive case management in a process that includes assessment, planning, facilitation, implementation, coordination, monitoring and evaluation of the member’s needs. This position will be in the office one day a week; the remainder of the week will be spent in clinics with patients.
