About this role

We have an exciting opportunity for an independent, autonomous Prescribing Clinician to play a key role in our Care Coordination and Integrated Urgent Care services across the Bath, Swindon and Wiltshire locality. We are seeking a motivated individual to join our team on a part-time, permanent basis, working regular weekend shifts. You will work across our Care Coordination service in hours and to support our Urgent Care service out of hours. Our Care Coordination shifts are based at our Chippenham Head Office and our out of hours sessions will be based at one of our Primary Care Centres.…
