POSITION SUMMARY Responsible for the assessment, delivery, supervision, and evaluation of speech therapy services to agency patients in accordance with the established physicians’ Plan of Care and in compliance with the applicable federal, state, and agency regulations, policies, and procedures. DUTIES AND RESPONSIBILITIES Completing comprehensive and OASIS assessments. Ongoing interdisciplinary assessment of the patient. Development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
