Pay Range: $48.13 - $72.20 A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education. Hospital Sisters Health System (HSHS) is seeking a Manager to join our Care Management team. The Manager-Care Management implements an organized care management program for assigned ministries. Promotes an interdisciplinary process that evaluates a patient's progress from admission through discharge. Ensures that patient's care and plans for discharge are provided in the appropriate setting and in a timely, cost-effective manner. Variations from expected outcomes are analyzed and information shared to further improve plan for discharge, utilization or resources, and quality of patient care.…
