Become a part of our caring community As a Community Health Worker, you will serve as a liaison between health and social services and the community, identifying health-related issues, collecting data, and discussing concerns. The region of service will be from downtown Atlanta to Decatur, GA. You will report to the Chief Medical Officer for the Atlanta Market. The Community Health Worker will: Visit patients in the hospital during admissions as well as assist in set up for post discharge appointments.
