Registered Nurse Case Manager

Job Description: The RN Home Services Case Manager utilizes clinical expertise and critical thinking skills to direct resource utilization to develop and implement a plan of care that is both cost effective and is designed to assist the patient in moving to functional independence and improve quality of life. The RN Home Services Case Manager is the primary caregiver upon admission, recertification, and discharge for the purpose of assessing progress toward stated goals. This role is responsible for the completion of the Outcome and Assessment Information Set (OASIS) and tracking progress against admission assessment.
