Job Description: We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team. In this role, you will provide direct patient care in the home setting under the direction of a Registered Nurse and in collaboration with physicians and other healthcare professionals. You will support patients and caregivers through skilled nursing services, patient education, medication management, and ongoing monitoring, helping patients achieve the highest level of independence and comfort in their own homes.
