Job Description: The Hospital at Home RN coordinates and evaluates complex care delivery projects, providing advanced clinical expertise, education, and contributing to research within their specialty. This role facilitates change and collaborates closely with Tele Hospitalists, Physicians, APPs, pharmacists, respiratory therapists, and patient placement teams to manage all aspects of hospital-level care at home admissions and transfers.
