Job Description: The Maternal Health Connection RN supports patients in remote or rural areas, enabling them to receive obstetrical care within their communities. This individual, local to the community, facilitates visits between patients and obstetrical care providers. The RN also delivers evidence-based health education programs in community settings, both virtually and in-person. Responsibilities include fetal monitoring, conducting biophysical profiles, lactation counseling, facilitating visits with and without providers (including home visits), providing health education sessions, customizing educational activities, implementing care plans from providers, attending community events, and coordinating resources based on patients’ individualized needs.…
