Community-Based Palliative Nurse Practitioner

The Palliative Medicine Nurse Practitioner provides consultation in palliative medicine to meet the needs of a patient as requested by their referring physician. The nurse practitioner is a specialist in assessing the patient's condition and developing a plan of care to improve patient understanding of their complex illness, adherence to course of treatment, and quality of life in context of patient’s values. The nurse practitioner also develops a plan of care to manage complex disease-related symptoms including emotion, physical, and existential components.
