We are currently seeking a dedicated and passionate Clinical Liaison Registered Nurse (RN) to join our team at KeystoneCare. This vital role involves acting as an advocate for patients within an acute care setting until they are safely discharged to their homes. The Clinical Liaison RN is responsible for generating referrals for home health, palliative care, hospice, waiver, and private duty services by building strong relationships with case managers and social workers.
