About this role
Our client, based in Solihull, is seeking an experienced Social Worker to join their busy Discharge to Assess Team. This is an exciting opportunity for a skilled Adult Social Care practitioner to support individuals transitioning from hospital settings back into the community, ensuring safe, timely, and person-centred discharges. Working within a fast-paced environment, you will undertake Care Act assessments, support planning, mental capacity assessments, and safeguarding work to ensure people receive the right support at the right time.
