Summary of Essential Responsibilities The APP will work closely with a physiatrist (a physician specializing in physical medicine and rehabilitation) to: Perform acute care consultations for hospitalized patients. Evaluate patients to determine the most appropriate level of post-acute care, such as: Inpatient rehabilitation, Skilled nursing facility, Home with therapy services, o ther rehabilitation settings Assist the acute care rehabilitation specialist with inpatient rehabilitation consultations. Collaborate with Mary Free Bed Rehabilitation Hospital physiatrists and the multidisciplinary care team to coordinate patient care.…
