About this role
The Community-Based Case Manager provides intensive substance use disorder case management, brief counseling, and care coordination for adults served by the Meharry Addiction Clinic. The position works primarily in the surrounding community and periodically in the clinic to connect clients with substance use disorder treatment, HIV/HCV services, insurance, housing, employment, and other supports and to promote continued engagement in care.
