About this role
The Peer Navigator connects patients with HCV, HIV, and sexually transmitted infection prevention, testing, treatment, and related services, including substance use disorder treatment, medication for opioid use disorder, behavioral healthcare, insurance, and patient-assistance programs. The role provides navigation, education, HIV/HCV testing, peer recovery support, appointment assistance, and encounter documentation in the electronic health record and TN-WITS. This is a fully in-person position primarily based on the Syringe Services Program mobile unit, with clinic and community-outreach responsibilities.…
