This role is responsible for identification of patients at nutritional risk, planning and implementation of medical nutrition therapy for assigned population, determination of nutrition diagnosis, development and implementation of evidence-based nutrition care plans, monitoring and evaluating the patient’s progress, and communicating with the interdisciplinary care team.
