Job Description Summary The House Calls Advanced Practice Provider (APP) is responsible for complex clinical management of patients in a home setting. An advocate for the patient, the APP will provide total care for the patient in collaboration with the House Calls Medical Director and leadership team. The APP will ensure that timely, patient-centered, appropriate evidence-based medicine is provided.
