Job Description: Position Summary Unit C3 is the leader of patient progression on the unit, coordinating progression and discharge activities to align with each patient’s target length of stay. In this role, the Unit C3 serves as the primary liaison between interdisciplinary team members on the unit such as the Unit Charge Nurse, Utilization Review nurse, Social Worker, Primary Nurse, PT, OT, and Primary Physician. Key Responsibilities: Acts as a spoke of the Hub by partnering with other Care Coordinators to facilitate patient progression and care coordination at a unit level to achieve system goals.
