Interacts daily primarily via telephone and electronic communication. Job Description: Essential Functions: Initiates telephone contact and completes a health risk assessment. Escalates complex or urgent needs to a supervisor. Connects patients to the right program once needs are identified. Develops individualized self-management skills to patients and families through education and resource provision.
