Overview: Job Description Summary: Ensures care continuity for patients and families and collaborates in the development, implementation, and evaluation of all program components. Job Description: Essential Functions: Develops, implements, and enhances special projects promoting inpatient/outpatient continuity of care and parent engagement. Ensures the provision of high quality, individualized, comprehensive, cost effective patient across the continuum. Initiates, tracks, analyzes, and manages all program quality improvement and research projects, program goals and objectives.
