Overview: Job Description Summary: Responsible for assisting patients and families in the coordination and implementation of their cardiopulmonary rehabilitation plan of care. In collaboration with other members of the team, the Cardiopulmonary Rehabilitation Coordinator ensures high standards of care, pertinent clinical education programs and cost-effective patient management plans. Job Description: Essential Functions: The Cardiopulmonary Rehabilitation Coordinator performs case management core functions.
