Location New York, New York Shift: Day (United States of America) Description: Health Advocacy to Support Quality Care: You Can Make A Difference Bring your talents to NewYork-Presbyterian and help drive the success of our Center for Community Health Navigation’s Health Promotion Program. The Health Promotion Advocate (HPA) supports NewYork-Presbyterian's emphasis on health equity by screening patient for social needs and, when needs are identified, connecting patients to resources to address their needs. HPAs also empower patients to understand how to enroll onto and to navigate our patient portal so that they can access their records, schedule appointments, and communicate with providers.…
