Warum wir Dich brauchen Jedes Jahr werden tausende Patient:innen aus der stationären Reha entlassen – und stehen dann oft alleine da. Mit der digitalen Nachsorge über myReha ändern wir das: Patient:innen setzen ihre Therapie zu Hause fort, begleitet von einem Therapieteam, das per Chat und Telefon erreichbar ist. Dieses Team bist du. Als DRV-Nachsorge-Therapeut:in bist du die Person, die Patient:innen nach der Entlassung auffängt, motiviert und durch ihre digitale Rehabilitation führt. Was Du mitgestalten wirst Patient:innen betreuen: Du begleitest Patient:innen nach der stationären Reha durch ihre digitale Nachsorge – per Chat und Telefon.
