Current referral volumes exceed 1,600 active referrals, including: Clinton Street Primary Care: ~500 Lung Cancer Screening (LDCT): ~184 Dietitian Referrals: ~633 Although Clinton Street is a smaller practice, it supports a complex mix of services, including Primary Care, Lung Cancer Screening, Resident Physicians, Faculty Providers, the Geriatric Dementia Clinic, and Nutrition Services, all of which generate significant referral and care coordination needs.…
