HOME HEALTH Licensed Clinical Social Worker - LMSW Schedule: 8:00 am – 5:00 pm, 3 days per week (Same 3 days each week between Monday - Friday) Location Office Location: Grand Rapids, MI 49546 Main Service Area: Grand Rapids and surrounding area This position requires daily travel to patient homes and residential facilities within your service area, with occasional office meetings.
