SUMMARY: The Home Care Services Manager is responsible for operational over-site of the home care PACE nursing assistants. In addition, the Home Care Manager is responsible for facilitating specified Interdisciplinary Team meetings. SPECIFIC DUTIES AND FUNCTIONS: Supervises and coordinates Home Care Activities at PACE Southeast Michigan (PACE SEMI), In collaboration with the Center Manager and IDT, facilitates the Participant-Centered Planning meetings, as well as complex Care Plan Development/Enrollment meetings, to assure consistency in care planning and service authorization for participants enrolled at PACE SEMI. Assists the Medical Director as requested in duties related to clinical and operational issues and special projects as may be required.…
